The report signup asks for identifying and contact information such as your name, email address, phone number if you request text messages, state, interests and consent choices. Do not submit medical records, diagnoses, insurance information or other sensitive health details through this form.

How information is used

NJ Weight Loss uses the information to deliver the requested report, operate the signup, prevent abuse and document your choices. Promotional email or text messages are sent only when you separately opt in to that channel. Report delivery is not conditioned on marketing consent.

Email and text choices

You may unsubscribe from marketing email using the instructions in a message. If you opt in to text messages, message frequency varies and message and data rates may apply. Reply STOP to opt out or HELP for help. Consent to receive marketing text messages is not a condition of purchase.

Service providers and sharing

Form hosting, email delivery, text delivery, website hosting and security providers may process information for NJ Weight Loss. Phone numbers and text-message consent are not sold or shared with third parties for their own marketing. Information may also be disclosed when required by law, to protect rights or safety, or in connection with a business transfer.

Retention and security

Information is kept only as long as reasonably needed for the purposes above, recordkeeping and legal obligations. Reasonable safeguards are used, but no internet transmission or storage system can be guaranteed completely secure.

Your requests

You may request access, correction or deletion where applicable, or withdraw optional marketing consent, by replying to a message you receive from NJ Weight Loss. Identity may need to be verified before a request is completed.
